How To Apply (Housing Choice Voucher Program)

To apply for the Housing Choice Voucher Program, you may do so from our website, pmha.us, or in our office, Monday – Friday 8:00am to 11:30am. When applying, we ask for birth certificates and social security cards for all family members. If you are a Veteran we also ask for a copy of your DD214.

When applying from our website, if we need any documentation, we will send you a letter asking you to respond within 2 weeks with information that is required. When applying in our office, we will inform you at the time of applying, what documentation you need to provide.

How To Apply
(Public Housing & Multifamily)

We take applications Monday - Friday between the hours of 8:00 AM -11:30 AM. To complete the application, we will need Birth Certificates and Social Security Cards for all household members, a valid ID for all adults in the household, and all adults must be present at the time of the application to sign all required documents. If the applicant is a Veteran, a DD214 will be required to receive the Veterans preference. PMHA offers applications online at pmha.us. Once the application online is received you will need to bring your documents in the Main office at 410 Court Street Portsmouth, OH 45662 during application hours to complete the application.

How To Apply
(Hisle Park)

Hisle Park is a needs-based program for young adults 18 – 24 years of age who have aged out of foster care, are homeless or are at risk of becoming homeless. For this reason, applicants are not chosen by date and time. We have units with multiple bedrooms to assist young adults with children.

 

The first step involves applicants completing an intake at Shawnee Family Health Center located at 901 Washington Street, Portsmouth, OH. There are no appointments needed to do so. Interested applicants can walk in anytime M-F from 7:30am to 4pm.

 

Individuals will complete an application for Hisle Park in addition to the intake. During the intake process at Shawnee Family Health Center, the clinician will complete a needs-based tool to help determine a level of priority. The Prioritization Committee will meet and vote on placement based on the current circumstances, diagnosis and the needs-based score each time there is a vacancy at the site.

 

Once approved by the Committee, the applicant will be referred to Portsmouth Metropolitan Housing Authority to complete the required paperwork. You will need birth certificates, social security cards, and proof of income prior to leasing.

How To Apply
(Booker T. Washington & Shawnee Apartments)

Booker T. Washington is a Section 811 property and Shawnee Apartments is a Section 202 property
available to adults 62 years of age and/or disabled. The first step involves applicants completing an
intake at Shawnee Family Health Center located at 901 Washington Street, Portsmouth, OH. There
are no appointments needed to do so. Interested applicants can walk-in anytime Monday – Friday
from 7:30 a.m. to 4:00 p.m.
 
Once approved by the Committee, the applicant will be referred to Portsmouth Metropolitan Housing
Authority to complete the required paperwork. You will need birth certificates, social security cards,
and proof of income prior to leasing.
